Religious Exploration: 2011-2012 Registration Information

Please read before filling out the form!

It is important for us to have contact information, the names of parents/ guardians, and knowledge of any medical conditions and allergies. It is also helpful if the we are aware of any learning styles or behavioral concerns, so that we can provide the best setting for your child.

Photo Permission

Photos are routinely taken during classes and special events.  These photos are used within the church to document the religious exploration experience.

We hope that our children and youth can also help document many aspects of church life so that it can be shared with newcomers on our website, in our newsletter, and in person. We need your permission to publish photos of your children in any way.  Please note that if photos of your children are placed on our web site, no names or other identifying information will be included

Please make sure you indicate your preferences in this matter by checking the appropriate box in the on-line registration form.

New for this year!   Emails for Youth 

You may give us an email address for each child you feel is old enough.  By giving us an a child's email address, you are giving RE permission to send the child occasional email messages. 

New for this year! Signing up to help out 

We ask that every family contribute to the RE program at UUCL. Together, we can create a wonderful program! Criminal background checks will be performed on all adults working directly with children/youth.
